Registration Requirements (Kindergarten-5th Grade)
CHILD MUST BE PRESENT TO BE REGISTERED
Acceptable Proof of Address
- Gas/Electric/Water Bill.
- Any document from the City Housing Authority or Human Resources.
- Special circumstance - if more than one family share the living space two documents of proof are required;
- Affidavit from the leaseholder/owner attached to Gas/Electric/Water bill.
- Any other legal document/bill showing parent/guardian(s) name and current address.
- Letter from lease-holder notarized (letters must state that parent/guardian and child reside with the lease holder), and letter from parent/guardian (letter must state that parent/guardian reside with the lease holder) both letters MUST be notarized. Bill from lease-holder.
- Letter from shelter.
- Original Birth Certificate/Passport/Baptismal Certificate.
- Proof of Immunization.
